Housing Policy for Unity Churches and Ministries-repository of student transcripts.
Scope
This policy applies to all Unity ministries offering classes that may be applied toward Unity credentialing.
Background
UUMS has successfully piloted this service with several ministries, including Titusville, Flint, New Orleans, and Sacramento. The following policy formalizes the process.
1. Eligibility
A church or ministry is eligible to house transcripts with UUMS if:
- The classes offered meet Unity educational standards.
- The church approves the instructor and meets Unity and UUMS minimum qualifications for teaching the selected course. Provide documentation.
- Class records are submitted accurately and in full.
5. Record Retention & Access
- UUMS will securely store all transcripts indefinitely, in accordance with institutional policy.
- Students may request unofficial copies through the UUMS Transcript Request System.
- Churches may not alter records once submitted; any corrections require formal documentation and appropriate resubmission.
2. Fees
- Transcript Housing Fee:
$45.00 per student per class (amount to be finalized by Administration) - Fees must be submitted when records are uploaded.
- Payment may be made via:
- UUMS online payment portal-Earmark payment “Housing Transcript- (Date/Church)”
- Church Check payable to Unity Urban Ministerial School
3. Required Documentation

Student Information
The following information must be submitted for each class:
Instructor Information
- Name
- Address
- Phone number
- Class location (physical or virtual)
- Dates the course was offered.
Student Information
For each student:
- Full legal name
- Address
- Phone number
- Attendance record
- Final grade or pass/fail status
4. Submission Process
Churches must submit all transcript materials through the UUMS Transcript Housing Upload Portal (recommended website addition), or by email to (re*******@**ms.org) until the portal is active.
Step-by-Step Process
- Instructor completes the Transcript Housing Form and submits documentation verifying current credential leader status.
- Instructor uploads the completed form and class roster.
- Instructor submits official attendance and grading records.
- Payment is submitted.
- UUMS Registrar verifies records and enters them into the student information system. A confirmation email is sent to the church and students.
