This email serves as a reminder for Unity Credentialed Leaders who may not have turned in their annual report, or who’ve recently updated their email address and did not receive this notice. If you’ve submitted your Credentialed Leaders Annual Report form and received an email acknowledgment, you may disregard this reminder.
Please access your unique Credentialed Leaders Form (for ministers, licensed teachers, and spiritual leaders) through this link: Credentialed Leaders Annual Report Form. After completing the form and clicking “Submit” at the bottom of the screen, you will see a message, “Your profile has been updated.” You will also receive an email within 24 hours letting you know that your form was received.
LUTs, please know that within two weeks of you filling out your 2022 Credentialed Leaders Form, an acknowledgement email will be sent to the person you listed as your LUT sponsor. In order for this to happen, you will need to include the name and current email address of your sponsor.
For those licensed or ordained in 2022, you will not need to fulfill the CEU requirement, so please note this on the online form. If you are retired and plan to teach SEE for credit, please plan to complete your CEUs. If you have any questions, email Denise Boehm at
